Alright, firstly some things, that you need for decapitation to work properly (actually these are a bit optional, but this is one of possible variations).
-pl000a.pmd should be a body
without a head.-pl000b.pmd should have a head in it.
-pl000c.pmd should be an absolutely empty file (originally it has Leon's face, but people usually combine it with pl000b.pmd and leave this file empty).
-pl000d.pmd should have hair in it.
-pl000e.pmd should be empty file (it's originally Leon's Jacket).
-pl000h.pmd should have a head+hair in it (these will fly away after the decapitation).
Process:
Decapitation happens -----> pl000a.pmd file stays where it was ----> pl000b.pmd and pl000c.pmd and pl000d.pmd disappear ----> pl000h.pmd appears and flies away.

pl000a.pmd file is a body with head in this mod.
But it
must be without head.
When a decapitation happens the pl000h.pmd appears and acts like it should while the whole pl000a.pmd file stays at it's place.
Solution:You simply need to cut head and hair off this pl000a.pmd model. And then transfer the head and hair to pl000b.pmd.
That not very hard but still needs some few modeling and rigging, so it's not a one-mouse-click fix.
I did it roughly in 2 minutes, it may have some smoothing problem, I just don't have much time to make a perfect fix, so just showing you the meaning.

This one is a lot more simple.
This time pl000a.pmd is Okay, it is without head.
But, pl000e.pmd has the head in it. And pl000e.pmd originally is supposed to have Leon's Jacket, so obviously it doesn't disappear after decapitation and stays where it was. We need this file to be empty while the pl000b.pmd (which is empty now) must have the head.
Solution: Swap pl000e.pmd and pl000b.pmd.
For both mods.
In that mod where the head comes off white, you just need to copy eida-hair.tga and ada-face.tga from pl0b into the pl00 folder, and also rename ada-face.tga to ada-face_s.tga, seems strange but pl000h.pmd has links to those textures.
You should use some program to view the models at least, cause without it is hard to do anything with them.
