As far as I know, you need 100% Accuracy and 0 Deaths. The exact time is different for each chapter, but it should be easy to get the S for time if run straight through without stopping for any side things and avoid fights when possible.

I believe it's 78%+ for accuracy, no deaths, and try to keep clear time at about 18 minutes and under.
