A few ideas on what could work to increase the difficulty in Remake 2. -Enemy Grab attacks instantly kill if succeeded (with obvious exceptions to like Mr. X's closeline which only knocks back and deals very minor damage even on hardcore). So if you're getting ripped part by two zombies or take a huge licker claw to the head, you're obviously dead.
- Real Time Inventory (with maybe the exception of during key item uses) so X can seem like a larger threat in the lobby of the RPD as well as making other enemies more dangerous with less time to switch between weapons.
- Restricted bag space to the original two rows.
-No Boards. They're a huge boon that's for sure, but that's why I feel like when talking about ramping difficulty, removing them would be best.
- Immortal Zombies. You can shoot them until they're downed for a little while, but the head cannot be destroyed. They can still act and do as they normally would otherwise.
These are just ideas, food for thought. I don't know if they're actually possible or atleast within reason as I am not a modder, but it would be a fun difficulty!

I would like that actually. I'd also like Mr. X to be able to go in any room. So no safety once you get to him. Especially if the item box is real time. I agree with all of your points too this would make the ultimate challenge.
As for immortal zombies, removing limb dismemberment would also be a good idea since everyone would just shoot the legs off and zombies would pose less of a threat.

For increase difficulty no dismemberment and no head shot, this are the 2 most used shots
